Community Partnerships
Physician Availability
Nurse Practitioner rounds daily M-F. The NP reviews labs, x-rays, visits residents in real time to provide medical care as needed, visits all new admissions within 24 hours and reports findings to the Medical Director.
Medical Director rounds weekly. The MD rounds to all residents needing care each week including all new admissions within 5 days of admit. The Medical Director reviews all nurse notes and meets with the Nurse Practitioner for resident updates and care planning.
Wound Care Physician rounds weekly. Our Wound Care physician partnership enhances our wound care program by providing regular treatment and oversight for resident wound needs.
Cardio-Renal Physician rounds multiple times weekly. Our specialized cardio-renal physician partnership delivers consistent, proactive care and close monitoring for residents with heart and kidney conditions.

These specialists come to Valley View and provide direct care in our residents’ home ensuring a comfortable location and ease of access to care.
Valley View also offers Hospice Services with provider of choice for residents. The Hospice teams provide patient centered care with many service options including music therapy, message therapy, chaplain services, end stage dementia assistance, and sensory care, such as weighted teddy bears or stress balls, to assist in comfort.
Valley View offers both in-house transportation and partnerships with community vendors to support resident access to appointments and community outings. Our dedicated driver is also a CNA, providing an added level of care and assistance.
